Banging my head! Multiple turbo failures

@dsiggi I wish and pray it's something simple that we've over looked. My last bill with ford was over 5k for 11 miles of fun. If it winds up being something simple it'll make a good camp fire story over beers. lol
Its terrible in the moment, but Crisis plus time = humor

Interested in the outcome here. Not a turbo expert but when things are hard to diagnose like this, it usually ends up being something dumb....... or something that isnt what it seems at the surface in my experience.
+1 / #Metoo.

I hope this reaches a definitive conclusion for OP, and I’m also interested to see what turns up.

Got the failed turbo today and tore it apart. Pictures below. looks to me like the shaft burnt up at this second small bearing. Journal bearing has spiral scaring i would think for the shaft spinning out of it. Looks like the shaft is burnt up where it stepped down to a smaller shaft sized and then split in half. The brass ghost shaped piece also has some discoloration like it got hot.

could this all add up to oil starvation?

I think you are overspeeding the turbine. Most turbo machines need some back pressure so the wheels stay loaded during an off throttle periods and sudden load changes. If I recall those turbo adapters are a restrictive office. Ford did not add that to reduce power, it was surge protection. Combine that with all the other mods, you have over speed. When you chop the throttle the compressor wheel side stops working, unloads, causing over speed. Ask your manufacturer about compressor surge. Also, the thrust washer is purple, symptom of compressor surge.

Dont the garrets need the optical sensor installed in the port to protect against overspin? Perhaps this is something missed?

WARNING: The maximum allowable turbocharger speed is 176krpm. The use of this product above 176krpm is at the owner’s risk, and can result in damage and premature failure. To protect the turbocharger from overspeed when operating, a speed sensor port is machined into the compressor housing for the fitment of speed sensor part numbers 781328-0003 (includes sensor and gauge), and 781328-0004 (includes speed sensor). Speed sensors sold separately.

I get the sensor, but what does it activate to slow the compressor? BOV? normally you would add inlet flow. With all the trash, maybe the BOV is stuck closed?

I think the tuner can monitor and adjust waste gate ?

I guess if the waste gate was erratic might cause some issues. But to move away from the compressor surge curve you increase speed or increase compressor inlet flow. Hope we hear the resolution.
