So, if you add the TPMS sensor to the spare, does the truck pick it up when you change the tire? Does the truck get confused because there are 5 sensors instead of 4? Inquiring minds want to know...

The truck will pick it up when you change the tires. Might take a bit (up to 50 miles) but it'll do it.
No, it won't pick up the 5th one in the spare position. Would have been better if it monitored the spare too.
As others have said, you'll need to get it balanced too.
Ah, but the question is, are all TPM equal? does it have to be a Ford one or is the signal universal?
