There is a little hole on the muffler and water is leaking from there and wonder if this is place there for a purpose?

To allow water, part of the combustion process to drain out. Otherwise you would have e a rusted out muffler in no time.

How about the rust on shaft end? Already rusted on 1000 mile! Is that normal for city driven truck?

Yes. It's a truck used in weather. Some lite surface rust will be expected. Is this your first car, ever?
Yea, first Raptor but not first truck. I had 2016 suburban and driven 110k miles and don’t recall any rust around same place. I have 2021 Tahoe 24k miles on it and just check that as well but no rust there too. That’s why I thought this is not normal.

Glad I checked this forum. I thought I had a hole in the muffler on my 2022. When I was putting a rear diff skid plate, so a little hole and some black residue and started to make an appointment at the dealer.
