Most spares are only provided by OEMs for emergency situations so you can drive yourself to the nearest tire shop. The spares usually aren’t rated for highway driving and aren’t meant for extended use. I’m not surprised Ford didn’t bother balancing or providing an extra TPMS. I’m just glad it’s a full sized wheel and tire.

You can get 4 sensors for around $50 on eBay. I will buy some for my 2019 next month and report back. Also no tool needed as it will sync up. Just drive for a few mins of driving. I will confirm this for the gen2 but this is exactly how it was for the gen1 trucks.

If the spare has a tpms, so 5 total, does that effect the truck?

No. The TPMS receiver is set to receive only 4 channels. There's an option in Forscan to set it to receive 6 channels (fur duallies), but even if you set it to 6, the dash display will only show the 4 mounted tires. And it may give an error if it doesn't get a 6th signal.

FWIW, I just rotated my spare in a few weeks ago. Haven't put a lot of miles on it, but haven't noticed any balance issues, either. Next time I'm out I'll try to get up to 65-75 and see how it does. Think I've been up to fwy speed since the rotation, but can't swear to it. Don't recall if I saw balance weights on it or not. Les Schwab may have balanced it for me though, they seem to treat me pretty good. Even though I had the spare in the back of the bed for them, they re-mounted the rotated-out tire back on the under-bed hangar for me.

Having put a flat spare on a car before, I'm definitely annoyed Ford cheaped out and doesn't include the ability to monitor the spare along with the other tires. But on the other hand, I am impressed it is a matching tire and wheel. Not something you see very often, but definitely nice to have if you are in the bush and need to use the spare to get out.
It's a wonder it has a spare at all. I can't believe how many include only a fix it kit.

Did you just drive the truck and the truck saw all 4 or did you have to use the tool?

Just drove it out. Before I went in I made sure 'auto-relearn' or whatever it's called was enabled in Forscan. By the time I hit the first stop after LS I had updated readings on all 4. It's in the spreadsheet in the Forscan-Tweak your truck thread. Just open the spreadsheet and do a search for "TPMS" and it will take you to the rows with the settings you need. NOTE: LS will NOT install a TPMS sensor you bring in, they insist on using their own "OEM equivalent" ones. Wasted a trip to the Ford parts counter before I knew that. Not sure if any others will put yours in or also insist on using their.s

Manual has a TPMS learning section, called reset procedures. page 399, where you reduce tire pressure after going into learning mode.

At first thought, I’m thinking it’s a good idea to rotate the spare in...

1. Prolonged tread wear by some percentage... is it 5% or 20%?

2. Keep tabs on the condition of the spare.

But maybe some negatives as well...

1. Cost... additional sensor, sensor installation, tire balance to get started, then need to purchase a 5th tire when new tires are needed, mount and balance 5th tire.

2. Additional time/work when rotating.

I don’t know...

I haven't confirmed it personally, but several posters have reported that the tool is unnecessary to reprogram the tire location. Move them and after five minutes of driving, the truck figures it out.

Myth confirmed Jamie.

I noticed after last service 2 out of 4 tires showed “--“ on the PSI. S/A insisted - no problem. drive it an within a couple minutes the truck will pick it up. It worked.
