I got an email from Ford saying I have an SYNC® 3 Navigation Map Update, it looks like it's $109.00 download. Is this really necessary? I'm just not sure I should do it or not.

I’ve never done the map upgrades. Now with CarPlay there’s really no reason IMO.

I did the Map Update on my 2019 with Sync 3.
I got the Map update from the link below
https://syncnavigation.com/ford/home

After putting in my VIN and the Serial number.... the map update was complimentary (free) for me.
It takes quite a while to down load the maps update to your desktop and same to pit on your USB stick. Then about an hour to install into Sync3.
I did mine and it went smooth and is working.
Just follow instructions carefully

Plug in your phone. Use Waze, Apple Maps or Google Maps. All three work and are updated regularly...at no cost to you, and with no work on your part other than Synching your phone. The days of built in navigation to a car about about over and done. That’s a near dead business.

I use Waze for everything and Apple Maps for backup. If they would just let us use the Nav directions in the gauge cluster like Sync that would be awesome.

I haven’t used the stock maps in the 18, the Apple CarPlay is badass. I used stock in the 14 raptor every day and to be honest Ford has badass mapping compared to others I’ve used.
I got a loaded ranger Lariat Fx4 a couple weeks ago and it only has the CarPlay setup, I’m not sure if the onboard maps was an option.

My carplay stops working fairly often and I have to unplug and replug the phone into USB to get it back. Kinda annoying but I don’t use nav much and music works over Bluetooth.

I’ve noticed it does that when the cable needs to be replaced. It’ll still charge but won’t give the signal. I have to replace mine every couple months, especially if you close the little door with the cable hanging out. I’m basically in mine 8hrs a day 5 days a week though.
I’ve tried Apple cables and the cheap cables from the gas station and they seem to last about the same time.
