Probably has something caught in the fan blades. Not likely a blade is broke. I've seen something as simple as a twig or leaf throw them out of balance.

I had this very issue with my Xterra (prior to the Raptor) after an oil change place pulled my cabin airfilters in which mice had made a nest. A small tree nut fell down in to the blower blade and it sounded like a V8 afterwards. I removed the blower assembly, took out the nut, reinstalled, and all was good.

Yeah that's pretty common, either there's a nest on it or its just full of debris and when you pull it out, stuff falls into the fan. Acorns and dog food get them pretty out of whack but the worst is a dead rodent thats been spun around till they come apart in the fan..
