2012 Raptor Transmission Noise / Issues

My brother's recently purchased 2012 Raptor is making a weird sound and "lurching" a bit when shifting to and from 4th gear. Here's a link to the video. The code it's throwing is: P0741. Has anyone experience a similar issue / sound? The truck has 200K+ on it, so it may just be due for a transmission.

He bought the truck about a month ago and had the transmission filter and fluid replaced then. The issue was happening before that, and has gotten worse slowly after.

Well, I had the po741. I replaced the square seal between the valve body and the case. I also replaced the separator plate between the 2 valve body halves, a shift solenoid, put in a cooler bypass, and shift improvement kit . That was 3000 miles ago and no problems since

I had the same issue with the engine revving like that, but it didn't make any noise. The revving was caused by the lockup torque converter unlocking. I had a bad solenoid. It couldn't hold the pressure to keep the converter locked. When I had it apart, I basically replaced what ograptor did. My guess is that the previous owner had that problem for a long time, and it eventually burned the clutch material for the torque converter clutch. It sounds like metal on metal to me. Did the person who changed the fluid say if there was a lot of shavings in the pan?
