I always knew it was a bloated, wasteful system, but I was blown away that it was THAT bad that you charge Medicare over double what it should cost. That's awesome that people that work there don't even care.
I know most people do not understand how Medicare “system” works, it is complicated to say the least. However, One thing to note is CMS does spend a significant amount of money for their Fraud and Abuse program to do their best to prevent, catch and enforce regulations. That person charging our government is the one abusing the system, not CMS.

Also, people you talk to on the phone when you call CMS, they are contractors, not government employees. The career feds are beyond passionate about what and how they do their work, otherwise none would work for the ridiculous salary they take home and grief they receive.

Maybe, that retirement deal is pretty sweet though, and zero chance of ever getting fired. ;)

Retirement maybe nice, if you are able to save enough money for it, and if you make it to retirement age while you still have some sanity left.

Also, there are worst things than getting fired.

SIL was career fed. Her financial advisor told her that her retirement was worth four mil. All she had to do was suck it for thirty years working with dipshirts. She hated the last fifteen but couldn't afford to quit. Worth it to her in the end I guess.

Does any Government employee care? As John said, where is the incentive to care?

In my younger days when I worked for the city government (Austin), it seemed there were basically 3 types of employees. There were those who were incompetent and didn't really care, but enjoyed the salary and virtue signally, so were intent to coast. Then there were those who didn't really care, realized it was a dead end job, and got the hell out as soon as possible (I was in this category). The last were the people who actually cared and tried to do a good job, but were so underappreciated and stretched thin by having to do their work and the work of employees in the other two categories, that just couldn't be that effective.

YMMV.
