I know I am late to the party on this and it may be a long shot. But I have only had one other customer that had issues with passing inspection and it turned out not to have anything to do with the tuning itself. It was a result of the installation of the headers. The shop that installed the headers got the O2 sensor plugs backwards and that caused them not to read properly. While this MAY NOT be the issue here, it is a real possibility and it does happen. It is also possible to damage the wiring for the O2 sensors, so if you ever have issues like this, while keeping the relearn and all that in mind, it will also pay to double check behind the shop to make sure that they installed everything properly and that they didn't damage wiring without noticing.
spent some time talking to Mike. Truck is going in next week to have the O2 sensors pulled and checked, he things it all originates with either a bad sensor or mis connected sensor.
